Social Work Case Manager (Lead) for Delta HealthPartners office located in Marks, MS.

Postion Summary

The Social Work Case Manager will work as part of a team to provide direct care coordination/case management services to pregnant women and infants in assigned service areas in any of the following counties: Bolivar, Coahoma, Quitman, Sunflower, Tallahatchie, Tunica, and Washington.  The Social Work Case Manager will perform administrative, program, and community outreach duties in the execution of job requirements.  The Social Work Case Manager will exercise leadership in identifying client needs and providing services to clients and members of the community is required.  The Social Work Case Manager is expected to provide services within the scope of practice as outlined by the Mississippi State Board of Examiners for Social Workers and Marriage & Family Therapists.  This is a full-time position.  This job requires travel.

Experience/Qualifications

A bachelor’s degree or master’s degree in social work from an accredited school of social work at an accredited four-year college or university is required.  A valid license to practice social work in the State of Mississippi at the BSW or MSW level and a minimum of two (2) years of work experience with a human/public health service agency, preferably with a maternal and child health background, are required. The applicant must possess a valid Mississippi driver license and professional liability insurance. Strong leadership skills and the ability to communicate and work with diverse populations at all levels, including clients served by the program, local health care providers, community leaders, community volunteers, school officials, local/state/federal elected officials and the media are required.
